New Feature - Create your own Secure Online Patient Forms / Questionnaires

We have just added the capabilty for you to be able to add your own Secure Online Patient Forms / Questionnaires.

You are able to ask 5 different types of questions:

- Patient can only choose one of the answer that you have provided (radio buttons)
- Patient can choose multiple answers from those that you have provided (tick boxes)
- Patient should type their own answer (small answer box)
- Patient should type their own answer (large answer box)
- Patient should type a date in the format dd/mm/yyyy

When a patient fills in your form/questionnaire a notification of a new request is sent to an email address of your choice. The results are kept in your secure logs.

This page will be https secure for your patients.

This would be perfect, for example, for you to add your own Travel Questionnaire or a New Patient Questionnaire. We had thought about adding these two forms for you but we found that different surgeries wanted to ask different questions. We decided to just let you add your own!
