Add my own online patient form/questionnaire

We have added the ability for you to add your own online patient form/questionnaire to your website.

To add your own online form/questionnaire login to your website management area, click on Website Content Management, click on Menu/Page Management, click Add a new page at the top, click the Predefined Page radio button, choose Questionnaire on the Please select the page you would like to add: drop down box, give the page a Name, and then click on the Add button.

Your new page will appear at the bottom on your left hand menu. To move the page to a different place in your menu click and drag the icon to the left of the page name. To edit the page click on the first icon to the right of the page name of a pen and paper.

To receive a notification email when a patient fills in your online form fill in the Email address field. You can choose for the emails to go to more than one email address.

To add text above the form click in the Text above the questionnaire: box.

To add text below the form click in the Text below the questionnaire: box.

To add text to the thank you message that is displayed when a patient fills in your form type in the Text to be displayed once the questionnaire is submitted box.

To start creating your online form/questionnaire first type the questionnaire name in the Questionnaire Name box, tick any of the the default questions that you want to ask, and click Create.

You can add further questions at the bottom of this page. First choose the type of question your want to ask and then click Next >>. You will then need to type your question and answers if the question type requires them. Add more answers by clicking on (+ add more). When you have created your question click Save Question. Your question will be added to the bottom of the list.

You can change the question order by clicking and dragging the icons.

You can make a question mandatory by clicking the grey star icon so that it turns red. Click again and it will change back to grey.

You can edit a question by clicking on the pen and paper edit icon. You can edit an answer by clicking on [edit].

You can remove a question/answer by clicking on the appropriate bin icon.

When you have finished creating your questionnaire click on << Back to Questionnaire page at the bottom of the page.

When you have finished creating the whole page click on your Publish website button and your new form will be available for your patients to start using.

When a patient fills in your online form, the details will be kept in your secure website logs under Your Own Questionnaires/Forms.
