Add my own online patient survey

Are you interested in what your patients think about your Surgery? Why not ask them by adding a survey to your website!
This feature was added in October 2009 after a suggestion from one of our customers. To add a survey onto your website login to your website management, click on Website Content management, click on Menu/page management, click to Add a new page, click the Predefined Page radio button, choose Survey on the Please select the page you would like to add: drop down box, give the page a Name, and then click on the Add button. Your new page will appear at the bottom on your left hand menu. To move the page to a different place in your menu click and drag the icon to the left of the page name. To edit the page click on the first icon to the right of the page name of a pen and paper. To add text above the form click in the Text above the survey: box. To add text below the form click in the Text below the survey: box. To add text to the thank you message that is displayed when a patient fills in your form type in the Text to be displayed once the survey is submitted box. To add a questionnaire first give it a Name
To add a question click on New Question. Type your first question in the Question : box and type your first answer in the Answer 1: box. To add another answer click on (+ add more). Once you have finished the question click on Save Question. You can add as many questions as you would like. You can edit an answer/question by clicking on [Edit] and you can delete an answer/question by clicking on the . You can also drag and drop the questions to reorder them by click on the .Once you have finished your questionnaire click on Publish website to make it visible to your patients. The results of your survey are kept within your Logs under Your Own Surveys (and GPAQ). The results are added together and displayed as a bar graph. You can reset the survey by clicking on Reset Questionnaire. You can add as many surveys to your website as you wish. If you want to add different types of questions and see the results individually for each patient then you can add a Questionnaire page rather than a survey page.
