Lightbulb New Feature - Patient signature required on your registration form
After a couple of suggestions from our customers you have 2 options with regards to needing patient signatures on your registration form.

1. Your patients fill out your registration form and send them to you online. You then ask your patients to come down to the surgery and sign their forms.

So you don't have to fill in a paper form yourself you can actually print a filled in form from your website management with some areas that need signing.

When a new request is made you get an email and the request is also kept in your secure website logs.

If you sign into your website management, go to logs, and go to your registration requests you will see your list of new requests.

If you click on view next to the patients name you can see a bit more about them.

If you click on view the filled in form you get a pop up displaying the form filled in with all of the patients details. If you print this filled in form you will notice there are 4 new areas that need filling in:
  • An area requiring the patients signature or a signature on behalf of the patient.
  • An area requiring a signature confirming consent to organ donation.
  • An area requiring a signature confirming consent to inclusion on the NHS Blood Doner Register.
  • And an area to be completed by the doctor.
You could ask your patient to come into the surgery to sign these new areas.

2. If you would like you can make your patients fill in your form, print it, sign it and then bring it down to your surgery.

Instead of a send button at the bottom of your registration form there will be a print button.

When the patient prints the form there will be 4 new areas that need filling in:
  • An area requiring the patients signature or a signature on behalf of the patient.
  • An area requiring a signature confirming consent to organ donation.
  • An area requiring a signature confirming consent to inclusion on the NHS Blood Doner Register.
  • And an area to be completed by the doctor.
The patient can then sign the 3 areas he/she needs to sign and then bring the form down to your surgery.

To change your form to print only you need to:

Sign into your website management

and click on options.

You will need to check the on radio button next to Registration Form only available to print.

Then save changes and publish your website. The requests will still be kept in your website logs.
